STUDIO REQUEST FORM

Terms and Conditions:

  1. The fully accomplished studio reservation form must be submitted to the Admin personnel 2 to 3 days before the date of usage.
  2. All requests are subject to approval and the Admin reserves the right to decline any reservations. 
  3. Cancellations and or changes must be sent in writing to the Admin e-mail at admin@lifegiverchurch.ph
  4. This reservation form must be for church activity only.
  5. Any damages must be reported to the Admin Personnel immediately.
  6. For the use of venue and equipment, Sanitation & Good Housekeeping must be strictly observed.

VEHICLE REQUEST FORM

Terms and Conditions:

  1. The fully accomplished Vehicle Request Form must be submitted to the Admin Personnel one (1) week before its usage.
  2. All requests are subject to approval.
  3. The vehicle is to be used for mission and other church-related activity/ies only.
  4. Any damages must be reported to the Admin Personnel Immediately.
  5. If an accident occurs, the applicant and the driver must be accountable to bear all costs of repairs or replacement if the situation demands. (Depending upon the result of the police report or written incident/accident report)

FINANCE CORNER

PAYMENT REQUEST

Terms and Conditions:
 
  1. This Payment Request Form is strictly for PRE-APPROVED items/services only. Call the Admin office before purchasing.
  2. Any purchase without pre-approval will not be paid or reimbursed.
  3. If approved, the fully accomplished Payment Request Form must be submitted to the Admin on or before Friday.
  4. All requests that are Php 1,000.00 and up are subject to Finance approval and may be released the next week.
  5. All requests of less than Php 1,000.00 are subject to Admin approval and may be released any time of the same week.
  6. The payment will be released exclusively to the authorized person only.
  7. The requestor must submit all supporting documents attached in this Payment Request Form. (Example, statement of account, Sales Invoice, Official receipt etc.)

BUDGET REQUEST FORM

Terms and Conditions:
 
  1. The fully accomplished Budget Request Form must be submitted to the Admin on or before Friday.
  2. All requests that are Php 1,000.00 and up are subject to finance approval and may be released the next week.
  3. All requests of less than Php 1,000.00 are subject to Admin approval and may be released any time of the week.
  4. Please submit all supporting documents 2 days after receipt of cash.

BENEVOLENCE REQUEST FORM

Terms and Conditions:
 
  1. The fully accomplished Benevolence Request Form must be submitted to the Admin.
  2. All requests are subject to finance approval.
  3. Benevolence form is strictly for Lifegiver Members and their immediate family only. Other churches or other people will not be entertained.
